Objectives: Clinical study evaluated the efficacy and safety of this technology for improving the appearance of upper arms lax skin.

Introduction: Synchronous Ultrasound Parallel Beam Technology safely coagulates the mid-dermis to maximize neocollagenesis and neoelastogenesis, while providing feedback-controlled energy deposition and skin cooling. The thermal injury leads to focal coagulation and cellular damage, initiating an inflammatory cascade that culminates in tissue remodeling and to lax skin improvement.

Materials / method: Multi-center, prospective, self-control clinical study was conducted in four US sites. 45 subjects completed 2 treatment sessions. 3 independent masked reviewers, who were not involved in the study, were asked to identify the post-treatment images, which were presented to them after randomization of the pre and post treatment images and to rank the upper inner arms skin crepiness/laxity grading scale. Objective measurements of the circumference and diameter at the maximal skin laxity point were taken at each of the study visits. Subjects ranked their improvement and their discomfort.

Results: 93% of treated arms showed improvement in upper arms lax skin appearance, as assessed (by the correct identification of the post treatment photograph) by at least 2 of 3 blinded evaluators. Upper arms circumference was significantly improved with mean reduction of -7.5±14.2mm, and the diameter of the upper arms was improved significantly by -4.5±7.4mm. Furthermore, 72% of the subjects reported on improvement. There was no device related adverse event. Most of the subjects reported none to mild levels of pain (with mean of 3.6±1.5 on 0-10 scale) during treatment and no discomfort afterward.

Conclusion: Synchronous ultrasound parallel beams treatment was demonstrated to safely improve upper arms lax skin appearance.

Objectives: This was a posthoc analysis to explore outcomes of treatment of glabellar lines (GLs) and lateral canthal lines (LCLs) with Relfydess™ (relabotulinumtoxinA [relaBoNT-A]) across different races, ethnicities and skin types.

Introduction: Relfydess (relaBoNT-A) is a novel, highly active, complexing-protein-free, ready-to-use, liquid botulinum toxin type-A1, which is authorized for marketing in Australia and in some markets in the European Union. The efficacy and safety of RelaBoNT-A for treatment of GLs and LCLs has been demonstrated in Phase III trials, with a duration of results through 6 months, and a rapid onset of effect, starting on Day 1.

Materials / method: For this subgroup analysis, data were pooled from 3 pivotal randomized, 6-month, double-blind, placebo-controlled Phase III studies treating GLs (NCT04249583), LCLs (NCT04249687), or GLs and LCLs in combination (NCT04247074). Subjects were divided into subgroups based on race (White vs. all other races), ethnicity (Hispanic/Latino vs. non-Hispanic) and Fitzpatrick skin type (FST I-III vs. IV-VI) for analysis of improvement in GL/LCL line severity (assessed live by investigators and subjects on the ILA and SLA, 4-point scales), subject satisfaction (FLTSQ questionnaire), and safety.

Results: Among 1012 pooled subjects (RelaBoNT-A=807; placebo=205), ~10% were of other races than White, ~14% Hispanic, and ~25% had FST IV-VI and ages ranged 21-81 years. In all race/ethnicity/FST subgroups, ≥1-grade ILA responder rates for RelaBoNT-A were high (Month 1: ≥97.4% GLs; ≥90.5% LCLs; Month 6: ≥40.4% GLs; ≥29.3% LCLs), and median time to return to baseline severity was >24 weeks. Subject satisfaction with GL/LCL appearance (FLTSQ total score) improved by a mean of 24 points or more at Month 1 after treatment, in all subgroups. Adverse events in all subgroups were mostly mild or moderate.

Conclusion: The posthoc analysis demonstrated similar high rates of severity improvement and satisfaction in all subgroups in both GLs and LCLs, affirming efficacy in subjects with diverse skin type, ethnicity, and race. The treatment was well tolerated in all subgroups.

Objectives: As with any surgical procedure, this technique is not free from complications [2]. Therefore the object of the present review is to assess the effectiveness of F.EL.C technology as a safe and non-invasive alternative to traditional surgical blepharoplasty for the cases in which the dermatochalasis severity allows to take into account techniques other than surgery.

Introduction: Blepharoplasty is a surgical procedure in which the eyelid skin […] is excised, redraped, or sculpted to rejuvenate the esthetic look of the patient along with correction of any functional abnormality [3]. The F.EL.C technology is delivered through a thin needle which creates a micro-voltaic arch without coming into contact with the skin. This produces nano-vaporizations limited to the stratum corneum, and it also stimulates the activity of the fibroblasts in the surrounding tissues thus activating a restoring process.

Materials / method: There is evidence in the scientific literature of different technologies that have been used to perform the treatment. The studies taken into consideration in the present review are those in which an RF based technology was the primary F.EL.C generator. All the patients reported in literature were affected by a mild upper eyelid laxity.

Results: The outcome was that the patients displayed tissue retraction that improved the laxity of the upper eyelids, resulting in aesthetic and functional improvement. The treatments were well tolerated and no side effects were observed [1]. In some patients a second or third session was needed to reach the expectations, also depending on the used technology.

Conclusion: Noted that the excess and laxity of upper eyelid skin affects more than 90% of women, the impact of these problems on the patient's self-esteem can become important enough to affect quality of life in psychological and sociocultural terms [3]. The data resulting from the literature review clearly indicates that offering a non-surgical alternative for the treatment of the dermatochalasis condition is not only clinically safe and effective, but it also has a significant impact on the patient’s psychological condition.

Objectives: The aim of the present study is to analyze the data from the published research in order to evaluate the effects of the combination of Intradermal RF and HA fillers for skin rejuvenation with respect to each methodology separately.

Introduction: Many scientific papers have been published in recent years analyzing the effectiveness and duration of HA filler treatments for skin rejuvenation and the use of RF based devices is widely known as an effective rejuvenation technique. Intradermal RF is a particular type of RF emission which is transmitted to the target tissue through a thin needle placed directly in the area undergoing treatment. The advantages of this technology are given by the possibility of adopting a selective approach, thus leading to a biostimulating effect exactly where needed.

Materials / method: The analyzed researches mainly focus on the following criteria:1)effectiveness of the treatment and durability of the results;2)safety of the treatment;3)possible effects of the RF technology on the active principles delivered through the filler.The studies were carried out on two groups:the control group received the “monotherapy” using only the HA fillers, while the study group received the combined treatments, first biostimulating the tissue using the intradermal RF, then injecting the filler.The results were analyzed according to the WSRS and GAIS scales (pic.A+B).

Results: The reviewed literature shows that: 1)the combined treatment allowed to achieve better and longer lasting results with a lower filler dosimetry with respect to the monotherapy; 2)the delivery of the Intradermal RF technology prior to the filler injection did not produce any side effects or discomfort to the patient, but it actually created a more favorable environment; 3)the delivery of the Intradermal RF technology prior to the filler injection did not produce any degradation of the active principles delivered through the filler.

Conclusion: The combination of Intradermal RF and fillers is a valid and safe procedure, able to boost results and duration of anti-wrinkle and skin tightening treatments. Moreover this combined treatment allows a reduction in the amount of filler administered, with a consequent reduction of possible side effects.

Objectives: To compare the G’ of a new hyaluronic acid (HA) injectable, Restylane® Shaype™ (HA-SHA), manufactured by the NASHA HD™ Technology, to other commonly used HA fillers produced with different manufacturing technologies.

Introduction: By examining the rheological characteristics of HA fillers, such as the elastic/storage modulus (G’), which reflects the gel strength/firmness, we can predict treatment outcomes. HA-SHA was recently approved by Health Canada and is a new HA injectable for chin augmentation, utilizing the innovative NASHA HD Technology with improved crosslinking efficiency and increased HA content. In a pivotal clinical investigation we have previously demonstrated that high G’ HA-SHA is safe and effective for correction of chin retrusion. This study compares the G’ of HA-SHA with other commonly used HA fillers

Materials / method: The G’ of 27 HA fillers from different brands were determined using an Anton Paar MCR 301 rheometer (Anton Paar, Graz, Austria) with a parallel plate geometry (plate diameter 25 mm). Measurements were performed in a sequence including a relaxation time of 30 min, a frequency sweep from 10 to 0.1 Hz at 0.1% strain. The frequency sweep was evaluated for G’, G’’, G* and tan delta at 0.1 Hz. The gap was 1 mm using a PP25 measuring system at 25°C.

Results: For all tested products, G’ ranged from 48 Pa for RHA1 to 916 Pa for HA-SHA. Hence, HA-SHA manufactured with NASHA HD, displayed the highest G’ across products manufactured with different technologies. G’ is considered an indicator of tissue lift and represents the energy stored in the gel and recovered once the shearing stress/strain is removed. Clinically, this would reflect the ability of the gel to resist compression caused by overlaying tissues and muscle contractions, providing ideal characteristics for treatment of the lower face, including chin and jawline.

Conclusion: HA-SHA, manufactured by the NASHA HD™ technology, exhibits the highest G’ among HA fillers produced with NASHA or OBT/XpresHAn technologies, and outperforms other frequently used HA-fillers produced with different technologies. The results of this study support HA-SHA as a suitable product for lower face shaping including chin augmentation.

Objectives: The objective is to develop a jawline assessment scale and establish its reliability and sensitivity for grading subjects in clinical trials and routine practice.

Introduction: Jawline definition is often considered important for enhancing a youthful and attractive appearance across genders. However, over time, this area commonly loses volume, leading to varying degrees of sagging. This change is due to a combination of bone resorption, fat repositioning, and tissue laxity. Validated clinical scales are needed to assess aesthetic improvement of jawline sagging after treatment with dermal fillers.

Materials / method: The Teoxane Jawline Assessment Scale (TJAS), a 5-grade photonumeric scale, was developed by clinical experts based on real-subject photographs and validated through photographic and live subject evaluation.

Results: The mean intra-rater weighted Kappa score between the 2 sessions of photographic validation was 0.86 using Cichetti-Allison’s formula, while inter-rater interclass correlation coefficient (ICC) was 0.93 for the combined sessions. The average intra-rater weighted Kappa score and inter-rater ICC for the live validation reached 0.79 and 0.86, respectively. In addition, evaluators identified clinically significant differences between photographs of subjects presenting a 1-grade or 2-grade difference in 85% and 95% of cases, respectively.

Conclusion: The TJAS can measure the degree of jawline sagging, detect a clinically meaningful difference of 1-grade, and is applicable to the ethnic diversity of the population. This scale is a repeatable, reproducible clinician-reported outcome and a useful tool for health care providers to classify jawline sagging both in clinical trials and routine patient care.

Objectives: In vitro evaluation of the capability of the tested product to protect an in vitro skin cell culture against toxic effects of oxidative stress and the activation of the autophagy system.

In vivo evaluation to demostrate the efficacy of the serum regarding brightness ans moisturing under normal conditions of use as well as its cosmetic properties.

Introduction: The intracellular process of autophagy is responsible for the removal of lysosomal vesicles containing lipofuscin, the pigment responsible for the yellowish-brown senescent appearance of the skin. To remove these unwanted pigments, a portion of the cytoplasm is first engulfed by a specialized organelle, the autophagosome, which then fuses with the secondary lysosomal vesicles and delivers the engulfed cytoplasm for degradation, improving the appearance of the skin by breaking down the lipofuscin(2).

Materials / method: The in vitro and in vivo studies were conducted to evaluate the efficacy of the product (SKIN GLOW [SERUM]): ethyl ascorbic acid (15%), potassium azeloyl diglycinate (1.5%), essential amino acids, 100KDaHA and hydrolyzed extract of Candida saitoana (0.1125%).
The in vitro study, was performed in a culture of human keratinocytes exposed to hydrogen peroxide (H2O2) to stimulate oxidative stress. Autophagic activity was measured by quantification of LC3-II protein (ELISA) and lysosome formation (fluorescent staining) after 24 and 48 hours of treatment with the serum at a concentration of 0.5%.

Conclusion: The in vitro study results suggest that the serum helps to activate the autophagy system so it protects our skin cells from oxidative damage, improving the skin aging.
The in vivo study shows that all the skin changes contribute to a more uniform and rejuvenated appearance of the skin. Subjective evaluations by the volunteers support the instrumental findings.

Objectives: To demonstrate that different biostimulators have different effects on remodelling of the extracellular matrix

Introduction: Diluted calcium hydroxylapatite (CaHA-CMC) and poly-L-lactic acid (PLLA-SCA) are injectable products that promote extracellular matrix (ECM) remodeling. This study is the first clinical comparison of the ECM remodeling effects and host tissue reactions of 1:1 & 1:2 diluted CaHA-CMC and PLLA-SCA

Materials / method: Injections of CaHA-CMC (diluted 1:1 and 1:2) and PLLA-SCA (5 mL) were administered to abdominal skin in 10 subjects. Biopsies were taken at 14 days, 3 months, and 6 months post-treatment for histological analysis of collagen, elastin, versican, and decorin. Host tissue response and ECM remodeling were evaluated using morphometric and histopathological methods. Statistical significance was assessed using mixed effects modeling and Tukey’s multiple comparisons post hoc test.

Results: CaHA-CMC demonstrated significantly higher collagen staining compared to PLLA-SCA at three months and persisting through six months. The increase in collagen was consistent across all groups but was most pronounced with CaHA-CMC. Elastin staining was significantly enhanced with CaHA-CMC treatments, whereas PLLA-SCA exhibited minimal elastin induction. Decorin and versican staining reached statistical significance at three months with CaHA-CMC but remained minimal with PLLA-SCA. Conclusion: CaHA-CMC (1:1 & 1:2) more effectively promotes ECM remodeling, increasing collagen, elastin, decorin, and versican staining compared to PLLA-SCA. Although PLLA‑SCA enhances ECM protein synthesis, its higher and sustained lymphocyte and GC response indicates prolonged immune response.

Objectives: The aims of this study were to evaluate, following Fx CO2 laser procedure, the skin re-epithelization kinetic and barrier associated clinical signs of a DC formulation containing vitamin B5, madecassoside and a prebiotic complex versus a reference dermocosmetic repair skin care (RDC).

Introduction: Fractionnated (Fx) CO2 laser procedures promote cutaneous resurfacing by removing the superficial epidermal layers which can help alleviate skin aging signs and reduce scars appearance. However, typical skin reactions such as erythema, desquamation, and crusts can frequently occur with a frequency and severity depending on laser settings. Therefore, post CO2 laser skin care with an appropriate dermocosmetic (DC) able to restore skin barrier integrity and improving physical cutaneous signs is recommended.

Materials / method: Adults with phototype II or III were included in this double-blind intra-individual study. Following Fx CO2 laser on similar zones of 4 cm² each on every subject’s back, DC or RDC were applied twice daily in the study center at a standardized dose of 2mg/cm² for 18 days (except Sundays). Clinical assessments included the wound healing kinetic overtime based on the re-epithelization score ranging from 0=no healing to 5=complete healing, individual scores of erythema, desquamation and crusts (all rated from 0=none to 3=severe), their composite score alongside standardized patients’ skin photos.

Results: This study was conducted on 25 subjects (15 women, 10 men), mean age of 37.7±7.3 years old, with phototype II (3;12%) or III (22;88%). The mean wound healing score was significantly (p<0.05) higher with DC compared to RDC starting from Day 6 till Day 14. On DC area, the composite score also showed a significantly (p<0.05) better improvement from Day 6 and a significantly (p<0.05) enhanced efficacy on crusts severity from Day 7 versus the RDC.

Conclusion: The post-CO2 laser application of the tested DC care provided a significantly faster and better repair efficacy associated with an improvement in physical signs, particularly crusts formation, compared to a reference dermocosmetic.

Objectives: To investigate the differential effects of commercial hyaluronic acid (HA)-based injectable dermal fillers (IDFs) on in vitro fibroblast activity, specifically examining their impact on extracellular matrix components synthesis and inflammatory/oxidative markers production.

Introduction: Injectable dermal fillers based on hyaluronic acid are widely used in aesthetic procedures. Beyond their immediate volumizing effects, these fillers may enhance tissue quality by stimulating fibroblast activity. However, different commercial formulations might elicit varying cellular responses, warranting detailed investigation of their biological effects.

Materials / method: Normal human dermal fibroblasts were cultured with four commercial HA-based IDFs (Juvederm Voluma®; Teosyal Ultra Deep®, Belotero Intense®, Agex Fill Ultra®). Measurements of six biomarkers were performed at 24 h, 48 h, and 72 h: type I and III collagen, elastin, TGF-β1 (extracellular matrix components/regulators), and IL-1β and 8-OHdG (inflammatory/oxidative damage markers). Supernatants from cultures were analyzed for these markers.

Results: Juvederm Voluma® showed the highest induction of type I collagen, while Agex Fill Ultra® led in type III collagen and elastin production. A positive correlation emerged between TGF-β1 and type I collagen levels (r=0.57, p<0.05). 8-OHdG showed negative correlations with both type III collagen (r=-0.41) and elastin (r=-0.46, p<0.05).

Conclusion: Different commercial HA-based IDFs trigger varying fibroblast responses in vitro, potentially predicting their clinical effects in aesthetic procedures. The study validates in vitro testing as an alternative to animal studies for evaluating IDF effects, offering insights into their biological mechanisms of action.

Objectives: 1. To examine the impact of adipocyte aging on facial aging.
2. To evaluate the effectiveness of high-intensity focused ultrasound (HIFU) in achieving fat reduction and SMAS layer tightening.
3. To assess the role of a slow-release injectable complex containing L-Carnitine and Vitamin C in addressing pathologies associated with aged adipocytes.

Introduction: Adipocytes play a crucial role in maintaining the structural integrity of both facial and body contours; however, the aging process of these cells significantly contributes to facial aging and cellulite formation. The progressive dysfunction of aged adipocytes leads to altered volume distribution and skin laxity. Advancements in aesthetic medicine provide promising strategies to address these changes. This study evaluates the combined use of a slow-release injectable complex containing L-Carnitine, which promotes fat metabolism, and Vitamin C, known for its antioxidant properties.

Materials / method: A systematic review of current literature was conducted alongside a clinical study involving patients presenting with facial aging and body cellulite. Participants underwent HIFU treatment, specifically targeting facial fat and SMAS layers to achieve localized fat reduction and tissue tightening. Following HIFU, a slow-release injectable complex containing L-Carnitine and Vitamin C was administered into the treated areas. Patients were evaluated at baseline and at 1- and 3-month intervals post-treatment. The assessments were performed using standardized photography, 3D imaging, and Ultrasound.

Results: Patients demonstrated significant reductions in facial fat volume. Visible results were noted as early as one-month post-treatment, with continued enhancement throughout the 3-month follow-up period. The combination of HIFU and the slow-release injectable complex of L-Carnitine and Vitamin C appeared to accelerate the reprogramming of aged adipocytes, resulting in both aesthetic enhancements and improved skin function. No significant adverse events were reported, and patient satisfaction rates were high.

Conclusion: This study highlights the efficacy of combining HIFU with slow-release injections of L-Carnitine and Vitamin C for the targeted reprogramming of aged adipocytes. This integrated approach not only reduces fat volume and tightens the skin but also addresses the underlying pathophysiology of adipocyte aging, offering a minimally invasive, effective treatment for both facial aging and cellulite. Continued research is recommended to refine treatment protocols and assess long-term effects.

Objectives: To present key data relating to the biostimulatory properties of poly L-lactic acid (Sculptra®, Galderma, Sweden; PLLA-SCA) regarding its ability to restore the various components of the skin, and hence their structure and function.

Introduction: The regenerative aesthetics field aims to restore the structure and function of damaged or diseased tissues. The aging process is one cause of tissue damage to the skin, resulting in the ongoing degradation of the extracellular matrix (ECM). Regenerative aesthetic treatments such as biostimulatory injectables can result in endogenous ECM regeneration. PLLA-SCA was the first commercially available PLLA biostimulatory injectable and is the most widely studied. Materials / method: Data relating to the biostimulatory properties of PLLA-SCA were collated from internal and external data sources. Seven global experts with an interest in the area of regenerative aesthetics reviewed the evidence base from the perspective of assessment of the effects of PLLA-SCA on the different components of the endogenous regenerative pathways involved in ECM remodeling. Subsequent discussion among the experts on the effects across the different components involved in ECM remodeling enabled a consensus to be reached regards the potential regenerative effects of PLLA-SCA.

Results: Injection of PLLA-SCA creates a scaffold within the skin, inducing a foreign body reaction involving protein adsorption, cell recruitment, fibrotic encapsulation and ECM formation. It directly affects dermal fibroblasts and stimulates production of type I (plus some type III) collagen. It also stimulates adipocytes, leading to improved skin quality. PLLA-SCA upregulates cytokines IL1b and CXCL6, which stimulate type I collagen synthesis, and stimulates elastin production, upregulates integrins, laminins and desmogleins, and upregulates TGF-β, which has a prominent role in tissue repair.

Conclusion: The biostimulatory effects of PLLA-SCA include fibroblast activation leading to stimulation of collagen, elastin and other proteins involved in ECM regeneration; adipogenesis and the expression of genes involved in collagen stimulation, inflammation and ECM remodeling. Clinically this presents as long-term improvements in the mechanical properties of the skin, restoration of volume and elasticity, increased skin thickness, dermal remodeling and other benefits. This suggests that PLLA-SCA stimulates endogenous regenerative pathways and has a key role in the regenerative aesthetic field.

Introduction: The aesthetic and reconstructive treatment of the buttocks has gained increasing popularity in recent years due to the use of minimally invasive techniques. Objectives: To evaluate the time course of GSH levels in skin cells post LCC treatment and investigate the efficacy of LCC in protecting skin tone from UV exposure.

Introduction: Glutathione (GSH) is a major antioxidant in cells which helps skin to protect itself against oxidative damage caused by free radicals and ultraviolet irradiation. LCC has been revealed as a GSH precursor bioactive from in vitro test, and it offers a high potential to act as depigmenting agent for the treatment of hyperpigmentation.

Materials / method: Normal human epidermal keratinocytes were treated with 100 μM LCC, with or without menadione, the levels of glutathione in keratinocytes were measured using GSH-Glo at baseline, 4 hours, 8 hours and 24 hours. Pigmented living skin equivalent (pLSE) models were topically applied with or without LCC at day 2, day 4, and day 6 after UV exposure. L* and melanin content, IL-1a and barrier protein were measured at day 3, day 5, day 7. RNA-sequencing of pLSE models was performed for whole transcriptome gene expression analysis at day 7.

Results: LCC rapidly boosted basal level of GSH in keratinocytes and significantly rescued oxidative stress. It remarkedly mitigated UVB induced pigmentation and inflammation, and further enhanced barrier protein expression levels against UVB exposure. The transcriptomics study revealed that LCC restored UVB dysregulated gene expression, the GO enrichment analysis demonstrated that LCC regulated genes were involved in multiple biological processes linking to epidermis development, differentiation and ECM organization.

Conclusion: LCC, as glutathione precursor active, effectively elevated GSH levels and protected skin from oxidative stress; LCC with topical application significantly protected skin against UVB exposure through multifaceted pathway, including antioxidant, anti-inflammation, barrier restoration and melanogenesis regulation. Applying topical glutathione precursor active in cosmetic products became a promising approach for better skin protection.

Objectives: Attendees will be able to compare the performance of the novel triple active combination formulation with other treatments and understand its unique advantages.

Introduction: While prescription-strength topical tretinoin is the gold standard treatment for photoaging, it can cause irritation, scaling, and erythema, thereby reducing its tolerability to users. Therefore, alternative retinoid and non-retinoid compounds, such as tretinoin precursors and other skin care ingredients, are commonly
used in cosmetic anti-ageing products to deliver similar clinical benefits without the tretinoin-associated adverse events.

Materials / method: This study aims to compare a new anti-ageing triple active facial serum containing retinyl propionate, niacinamide, and hexyl-resorcinol to a leading marketed anti-aging product containing Tripeptide-32. The study was an 8-week, double-blind, randomized, split-face clinical trial. Participants were Chinese women aged 40-60 years with lines, wrinkles, and hyperpigmentation. Expert visual assessment
of lines/wrinkles assessment on the 6 facial areas were evaluated at baseline and through eight weeks application, in addition to Antera 3D image analysis on wrinkle length and depth.

Results: Although both products demonstrated a statistically significant improvement from baseline condition at all timepoints, the novel triple active serum demonstrated greater benefit over the leading marketed
peptide product on several types of wrinkles, including perioral, side cheek, and Crow’s feet as determined by expert visual grading. These findings were further corroborated by image analysis of
wrinkle length and depth, where the triple active serum demonstrated statistically significant improvements of these attributes over the marketed tripeptide product.

Conclusion: This study demonstrates that the novel triple active facial serum containing retinyl propionate, niacinamide, and hexyl-resorcinol is more effective at reducing the appearance of facial lines and wrinkles than a leading marketed product containing Tripeptide-32.

Objectives: To determine the in vivo local tissue response and degradation of the poly L-lactic acid biostimulatory injectable Sculptra® (PLLA-SCA) and the biostimulatory filler calcium hydroxylapatite (CaHA-R; Radiesse®).

Introduction: PLLA-SCA and CaHA-R stimulate collagen synthesis. Various pathways lead to collagen stimulation, such as inflammation, which is connected to a fibrotic state, and regenerative pathways that occur when building blocks such as elastin are present. Quantitative data suggest PLLA-SCA is associated with increased gene expression for remodelling/morphogenesis to a greater extent than CaHA-R, while CaHA-R is associated with increased expression of more inflammatory genes than PLLA-SCA. This study evaluated local tissue response and degradation following subcutaneous implantation of PLLA-SCA and CaHA-

Materials / method: Thirty-two rabbits were implanted subcutaneously with 0.2 mL of PLLA-SCA, CaHA-R (both reconstituted as per manufacturer’s instructions) or negative control on the dorsal surface. At 4 days (baseline) and 2, 12 and 26 weeks, the tissue at the implant sites and regional lymph nodes were collected and processed for immunohistopathology and to evaluate local tissue response and product degradation. Evidence of local tissue response was determined using a microscopic reactivity score compared with the negative control, and persistence (degradation) was evaluated using a semi-quantitative scale.

Results: Average reactivity scores vs control indicated slight reactions for PLLA-SCA (3.1, 6.2 and 6.0 at 2, 12 and 26 weeks, respectively) and for CaHA-R at all time points (4.5, 5.6 and 5.3 at 2, 12 and 26 weeks, respectively). PLLA-SCA and CaHA-R had average persistence scores of 2.6 and 2.2, respectively, indicating faster degradation with CaHA-R. Immunohistology showed the presence of microspheres in CaHA-R but not PLLA-SCA samples, which persisted over time despite gel degradation. There was no evidence of PLLA-SCA, CaHA-R or any degradation products in the draining lymph nodes over 26 weeks.

Conclusion: These results suggest that the in vivo local inflammatory tissue response observed with PLLA-SCA was similar to that seen with CaHA-R. The reduced persistence, and hence greater degradation, observed with CaHA-R versus PLLA-SCA might result in a reduced duration of effect for CaHA-R in the clinic. The immunohistological presence of microspheres in CaHA-R samples persisted over 26 weeks despite gel degradation and this was not observed with PLLA-SCA, which was rapidly absorbed into the tissue.

Objectives: Here, we highlight how calcium hydroxylapatite (CaHA) microspheres may enhance skin quality by boosting the expression of versican and lumican, thereby improving the ECM and the normal functions of the dermis.

Introduction: Changes in skin appearance, such as wrinkle formation and decreased elasticity are linked to alterations of the dermal extracellular matrix (ECM). The general aging process involves the breakdown of elastic fibers and the disorganization of collagen fibrils. To maintain dermal integrity, proteoglycans serve as fundamental components in the formation of complex network structures. Versican is associated with the stabilization of elastic fibers, while lumican is important for the organization of collagen fibril assembly.

Materials / method: Primary normal human fibroblasts (NHDFs) were cultured with CaHA microspheres (isolated from Radiesse, Merz Aesthetics GmbH) at 1, 3 or 5 mg/ml for 72h or 168h. mRNA expression of lumican & versican were analyzed by qPCR. To assess lumican expression, cell lysates were generated using NP-40 buffer supplemented with SDS and analyzed by western blot. Immunofluorescence was performed on NHDFs incubated with CaHA microspheres for 168h and the samples were stained with antibodies targeting versican and lumican.

Results: CaHA microspheres co-cultured with NHDFs increase mRNA expression of lumican in a concentration-dependent manner. These findings were confirmed by immunofluorescence analysis, indicating that lumican and versican expression was upregulated by increasing CaHA microsphere concentrations. Compared to untreated samples, lumican protein analysis exhibited increased expression level in CaHA-treated samples. Additionally, co-staining of lumican and versican revealed induced signal intensity for both proteoglycans and fiber enrichment in areas with CaHA microspheres.

Conclusion: The current study demonstrates that CaHA microspheres effectively upregulate the expression of versican and lumican in vitro, key proteins for extracellular matrix maintenance. Our findings suggest that CaHA microspheres provide significant benefits in preserving elastic fibers and collagen fibrils by increasing proteoglycans as structural support.

Objectives: Here, we provide new insights into the molecular mechanism by which calcium hydroxylapatite (CaHA) microspheres stimulate elastin neogenesis. This process involves the necessary structural proteins, namely fibulin-5 and emilin-1, which contribute to the correct assembly of functional elastin fibers.

Introduction: During aging, elastic fiber assembly is highly downregulated, which is associated with progressive skin loosening and sagging. Effective elastin neogenesis requires more than just upregulating the base molecule, tropoelastin; it also necessitates proper alignment and development of functional elastin fibers.

Materials / method: Primary normal human Fibroblasts (NHDFs) were cultured with CaHA microspheres (isolated from Radiesse, Merz Aesthetics GmbH) at 1, 3 or 5 mg/ml for 24h, 72h or 168h. mRNA expression of elastin was analyzed by qPCR and supernatants were analyzed for elastin expression by ELISA. Cell lysates were generated using NP40-Buffer supplemented with SDS and analyzed by western blot for the expression of fibulin-5. For immunofluorescence, CaHA microspheres were cultured with NHDFs for 168h and stained with antibodies targeting emilin-1 and collagen 1.

Results: mRNA expression and secretion of elastin is up regulated by co-culture of fibroblasts with CaHA microspheres. Fibulin-5 protein expression is upregulated in a time-dependent manner by CaHA microspheres. Co-staining of collagen I with the elastin adapter protein, emilin-1, reveals distinct fiber enrichment at areas with CaHA microspheres. Maximum intensity projection of emilin-1 and collagen 1 indicates a network for both that forms around and in close proximity to CaHA microspheres.

Conclusion: Direct contact of fibroblasts to CaHA microspheres induces in a time- and dose-dependent stimulation of elastin-related structural proteins, fibulin-5 and emilin-1.
Our findings provide mechanistic insight of how CaHA microspheres support the maintenance and development of functional elastic fiber formation.

Objectives: Our research aims to introduce a novel peeling system designed to inhibit melanogenesis and safely reduce hyperpigmentation over a shorter time.

Introduction: Hyperpigmentation, caused by disruptions in melanogenesis, can substantially affect an individual’s quality of life. While the cosmetic industry has advanced treatments for dark spots, many current solutions often cause adverse effects with extended use, limiting their long-term clinical utility. Our research addresses this need by evaluating a novel peeling system designed to inhibit melanogenesis and safely reduce hyperpigmentation over a shorter time.

Materials / method: In vitro studies assessed the ability of the novel moisturizer to inhibit ultraviolet-A (UVA)-induced melanin accumulation in human melanocytes. In a single-center, controlled study, we assessed the efficacy of the peeling system in 33 healthy female participants aged 30–55 years with moderate-to-severe hyperpigmentation over a 6-week treatment period. Skin condition was assessed using clinical photography, 3D skin topography, and clinical expert evaluation (CEE) at baseline and 6 weeks posttreatment.

Results: The novel moisturizer demonstrated promising inhibitory effects on UVA-induced melanin accumulation in human melanocytes. Additionally, the single-center study, revealed significant visible improvements in skin pigmentation, brightness, tone, and texture with minimal adverse effects. After 6 weeks of topical treatment, there was a significant reduction in the total number, area, and perimeter of dark spots compared with baseline. A significant improvement in skin homogeneity and reduction in skin contrast was observed at week 6 compared with baseline.

Conclusion: In conclusion, the study introduces an innovative peeling system designed to target hyperpigmentation at the cellular level by combining a synergistic blend of acids and brightening and penetration-enhancing ingredients for a comprehensive solution. The convergence of in vitro and in vivo results reinforces the efficacy of the innovative peeling system in addressing moderate-to-severe hyperpigmentation in healthy females.

Objectives: This study aimed to evaluate and compare the efficacy and safety of a new 2-MNG containing dermocosmetic serum versus Cysteamin 5% in the management of melasma.

Introduction: Melasma is a common acquired disorder of hyperpigmentation that is characterized by irregular light to dark brown patches on the face. Its pathogenesis is not yet completely understood, although there are some known triggering factors such as sun exposure, pregnancy, hormonal changes etc. There is also a clear genetic predisposition. Initial treatment typically involves topical solutions. Though there are standard treatments available, they are usually not prescribed for long term and also pose a risk of recurrence once the treatment is discontinued.

Materials / method: 140 participants with melasma, ranging from mild to severe based on Investigator Global Assessment (IGA), were enrolled for a 4-month treatment. Following a 2-4 weeks wash-out period, baseline assessments (digital imaging, dermatological examination, and spectrophotometer readings) were performed. Subjects were then randomized into either the serum tested product (TP) or marketed product (MP) group, ensuring balanced severity. Follow-up assessments included modified Melasma Area and Severity Index (mMASI), IGA, Investigator Global Assessment of Improvement (IGAI) and other skin attributes.

Results: After 2 months of treatment, assessments revealed significant reduction in melasma severity in mMASI and IGA for both groups compared to baseline with no significant difference between groups. In terms of IGAI, 98% of subjects using TP improved slightly and moderately compared to 85% with a difference of 30% in favor of TP, for those who improved moderately, which showed statistically significant at month 2. Skin attributes show significant improvement compared to baseline for both groups, with a significant difference at Month 2, in favour of TP in skin tone, smoothness and brightness.

Conclusion: After only 2 months of treatment, both products demonstrated significant improvement in melasma. However, 30% of subjects using TP showed a significantly greater improvement in melasma severity compared to those treated with MP at month 2. Additionally, the TP exhibited statistically superior improvements in skin attributes such as smoothness, evenness, and brightness compared to the MP.

Objectives: Here, a serum containing 2-MNG, an ingredient that quenches melanin precursors has been tested in post-acne PIHP.

Introduction: Post-acne post-inflammatory hyperpigmentation (PIHP) is a frequent condition especially in patients with darker phototypes and has a significant impact on quality of life. Depigmenting topical treatments are used to reverse the condition and are most often based on tyrosinase inhibition. Here, a serum containing 2-MNG, an ingredient that quenches melanin precursors has been tested in post-acne PIHP.

Materials / method: A single arm, full face, 3 months study, has been conducted in Mauritius from April to August 2023 to evaluate the effectiveness of a new depigmenting serum, applied twice a day, as a standalone procedure i.e. without any recommended sunscreen. Subjects had to have mild acne on the face and moderate to severe PIHP. Clinical evaluations were conducted using PAHPI score, mean darkness of PIHP lesions, Global Acne severity and acne lesion counts. Colorimeter measurements were conducted on PIHP lesions and normal skin. Subject Global Asses, local and global tolerance, stigmatization questionnaire.

Results: The study included 32 subjects with 97% females (mean age 31 years) of diverse phototypes (50% IV, 44% V and 6% VI). PAHPI score significantly decreased from D56 with a 15.8% decrease at D84. The PAHPI lesion intensity sub-score displayed a decrease of 19.7% at D84. The mean darkness clinical evaluation significantly decreased from D28 with a decrease of 25.1% at D84. We observed a significant 60% reduction of the contrast between lesional and adjacent areas at D84. This improvement was observed since D14. The stigmatization questionnaire showed a 30,6 % decrease of the total score at D84.

Conclusion: This study demonstrated the efficacy and tolerability of the serum in the treatment of post acne PIHP as a standalone skincare. Good clinical results are confirmed by the objective measurement of the ITA and Delta E. Cosmeticity was excellent and will help observance in real life. The reduction of the stigmatization score illustrates the impact of PIHP and its improvement over time. These results can be considered highly encouraging with the new serum, as in real life, its use in combination with a UVA/visible light filtration sunscreen would probably increase patients benefit.

Objectives: Here, a novel serum containing 2-MNG, an ingredient that quenches melanin precursors (Routine A) has been compared to a tyrosinase inhibitor containing dermocosmetic serum (Routine B). Both serums were used in conjunction with a broad spectrum SPF50+/UVA sunscreen.

Introduction: Acne-induced post inflammatory hyper pigmentation (PIH) is a frequent, difficult to treat, condition. Topical treatments used to reverse the condition are most often based on tyrosinase inhibition with either drugs or dermocosmetics. Here, a novel serum containing 2-MNG, an ingredient that quenches melanin precursors (Routine A) has been compared to a tyrosinase inhibitor containing dermocosmetic serum (Routine B). Both serums were used in conjunction with a broad spectrum SPF50+/UVA sunscreen.

Materials / method: A single center randomized, double blind, parallel group, 12 weeks study has been conducted in one center in Mauritius from February to October 2023. Subjects had mild acne on the face and moderate to severe PIH with a PAPHI score >10. Patients had to apply both serums at home on the face twice a day in the morning and in the evening. The sunscreen had to be applied in the morning and at the beginning of the afternoon. Evaluations conducted at D0, D28, D56, D84 included PAHPI score, mean darkness of PIHP lesions, local and global tolerance and cosmetic questionnaires.

Results: The study involved 77 subjects including 92% females (mean age 30.5 yo), with phototypes IV 27%, V 68% and VI 5%, and from Indian 35.7%, African 29.9%, Mixed 34.5% descents. Both routines showed a significant decrease of PIHP severity at D84. The percentage of improved subjects rated by investigators at D84 was higher with routine A. Routine A showed a faster and significantly greater decrease in the PAHPI number of PIHP lesions sub-score. The two routines displayed a significant and similar improvement on mean darkness. 60% of subjects in group A perceived that their PIHP were improved.

Conclusion: Both routines showed very good cosmetic acceptability, and were very well tolerated. However, routine A showed better outcomes on fine lines, wrinkles, pores and skin texture.
This study shows that both routines comprising innovative depigmenting serum and a broad spectrum SPF50+/UVA sunscreen are effective on PIHP with a twice daily application over 3 months. The patients’ self-perceived level of improvement suggests a clinically relevant effect from both routines. Routine A, showed better efficacy on several secondary criteria related to PIHP and cosmetic outcomes.

Objectives: Here, an innovating serum containing 2-MNG, an ingredient that quenches melanin precursors, twice a day (group A), has been compared to HQ4% (group B) in the treatment of facial melasma.

Introduction: Hydroquinone 4% (HQ4%) and Kligman’s trio are the gold standard of melasma treatment. Very few cosmetic formulas have claimed equivalent efficacy vs HQ4%. Here, an innovating serum containing 2-MNG, an ingredient that quenches melanin precursors, twice a day (group A), has been compared to HQ4% (group B) in the treatment of facial melasma.

Materials / method: A 3 months comparative, randomized, investigator blinded, parallel group study, has been conducted in Brazil from August to March 2023. Both groups applied the same tinted high SPF, UVA PF and VL-PF sunscreen at least in the morning and at midday. Subjects had to have facial epidermal or mixed melasma of any severity. Evaluations were conducted at D0, D28, D56, D84 utilizing mMASI, local tolerance severity (0 to 5 score, from “no” to “very severe” signs of local intolerance) and the PUSH-D stigmatization questionnaire. The study included 109 female subjects (mean age 43.2 years, range 30-51 ye

Results: Both groups showed a significant decrease of mMASI at D84 (28% groupA, 34% groupB) with mean change from baseline of -2.16 and -2.56. This improvement was significant since D28. No statistical difference was observed between the two groups at any visit and the non-inferiority, with a predefined threshold of 1.3, was demonstrated at D84. Both groups showed a significant decrease of the PUSH-D total score at all visits with a significant difference between groups at D28 and D56 in favor of the serum. Patients receiving HQ4% significantly experienced more local irritations at D28 (6.0% vs 21.4%).

Conclusion: This study shows that a new depigmenting serum is non-inferior in efficacy than HQ4% on facial melasma over 3 months of use. The serum was better tolerated than HQ4%. Given the body of literature showing efficacy of HQ4% in melasma, this study strongly suggests that the new melanin quencher containing serum is an effective solution for melasma in association with a sunscreen efficiently covering UVAs and visible light, with optimized cosmetic properties and no risk of irritation and ochronosis opening the door for longer term use.

Objectives: To present in vitro characterization of the novel, liquid, ready-to-use, type A1 botulinum toxin (BoNT-A1), relabotulinumtoxinA (relaBoNT-A), including BoNT-A amount and activity.

Introduction: Most BoNT-As for aesthetic use, e.g. treatment of glabellar lines (GLs), are produced using precipitation for initial purification and lyophilization to stabilize the final drug product, and thereby need reconstitution before use. RelaBoNT-A is manufactured using the Precipitation-free Extraction and Activity-preserving, Refined Liquid (PEARL™) technology, from a proprietary C. botulinum type A1 strain. Each production step keeps the core BoNT-A protein in liquid suspension to maintain its original conformation, minimizing any denaturation or unfolding to preserve its neuromodulator activity.

Materials / method: Specific enzyme activity was determined throughout relaBoNT-A drug substance manufacturing as LD50 mouse potency units/mg BoNT-A1. RelaBoNT-A drug product was then characterized in vitro and compared with commercial onabotulinumtoxinA (onaBoNT-A) using three measures: BoNT-A amount by ELISA per GL on-label dose; SNAP-25 cleavage by a fluorescence resonance energy transfer-based assay (BoNT-A enzyme activity, BoTest®); SNAP-25 cleavage by a cell-based assay using human motor neurons from baseline to hour 72 (BoNT-A potency: binding, internalization and SNAP-25 cleavage within cells).

Results: The specific BoNT-A1 activity remained consistent for relaBoNT-A in the four fractions sampled throughout the drug substance manufacturing. In the final drug product, relaBoNT-A contained more BoNT-A per on-label GL dose (0.27 ng) than onaBoNT-A (0.18 ng), and had a higher enzyme activity (53 vs 29 BoTest units) per GL dose, and a higher specific activity per pg BoNT-A, with onaBoNT-A displaying 81% of the specific activity of relaBoNT-A. The cell-based assay showed a higher potency and earlier onset of SNAP-25-cleavage for relaBoNT-A than onaBoNT-A after incubation with on-label GL doses.

Conclusion: PEARL technology preserves a high specific activity for relaBoNT-A throughout the manufacturing process. The final relaBoNT-A drug product has a higher amount of core protein per GL dose, higher enzyme activity (SNAP-25 cleavage) per dose and per pg core protein, and higher potency and faster onset in a cell-based assay per GL dose than onaBoNT-A. This indicates a high protein quality for relaBoNT-A, and may help explain the Phase 3 clinical data, showing an onset starting from Day 1 and a median duration of effect of about 6 months in GLs and lateral canthal lines.

Objectives: This updated global advisory board was designed to revisit the skin quality emergent perceptual categories (EPCs) definitions, integrate new research and technology, provide a template algorithm for achieving good skin quality by a range of products, and standardize assessment and treatment protocols.

Introduction: Skin quality is a broad and inclusive term sub-categorized into 4 emergent perceptual categories (EPCs): skin tone evenness, skin surface evenness, skin firmness, and skin glow. The EPCs were agreed by a consensus expert panel inspired to better guide defined clinicians in individualized patient care using non-surgical treatments and achieving optimal outcomes in dermatological practice.

Materials / method: A panel of 12 skin quality experts from North America (NA), Latin America (LATAM), Europe the Middle East and Africa (EMEA), and Asia Pacific regions (APAC) participated in a 3-week-long virtual advisory board in September 2024. The collaborative effort brought a diversity of experience able to thoroughly evaluate a range of older and emerging aesthetic products, including biostimulators, dermal fillers, and energy-based devices, enabling for a comprehensive regimen for skin enhancement.

Results: •Across sessions, there was a strong focus on patient-centered treatment plans according to specific EPCs, utilizing a variety of modalities tailor-made for each patient scenario.

Objectives: To understand the mechanism of action for MFU-V.

Introduction: Regenerative aesthetics aims to restore youthful skin structure and function to skin and deeper tissues by leveraging the body's natural regenerative processes. Unlike implanted biomaterial fillers, energy-based devices (EBDs) trigger aesthetic improvement through the body's physiological response to treatment. This narrative review assessed evidence on microfocused ultrasound (MFU) in regenerative aesthetics.

Materials / method: A systematic search of PubMed, Scopus, and Web of Science identified relevant peer-reviewed articles up to July 2024. Two independent reviewers extracted and analyzed data, resolving discrepancies by consensus. Findings were synthesized narratively, emphasizing MFU's mechanism of action, efficacy, and safety.

Results: MFU-V emits ultrasound energy that penetrates multiple skin layers and the superficial musculoaponeurotic system, creating localized thermal coagulation points. These points trigger biological responses that recruit fibroblasts and stimulate the production of new collagen and elastin fibers. Enhanced extracellular matrix (ECM) protein synthesis results in significant improvements in skin biomechanics and quality. These findings are supported by clinical evidence, showing improvements in skin firmness and texture following MFU-V treatment.

Conclusion: In summary, MFU-V operates through mechanisms involving TCPs that induce collagen and elastin fiber regeneration, resulting in noticeable improvements in skin structure, firmness, and elasticity. While MFU demonstrates a capacity to remodel the extracellular matrix and improve skin biomechanics, further research is needed to fully elucidate its long-term impact and optimal use within the field of regenerative aesthetics, within diverse patient demographics, and in combination with other injectable treatments.

Objectives: The objective of this prospective, randomized, blinded evaluator, multicenter clinical study was to evaluate the safety and effectiveness of RHA4 versus an FDA-approved active comparator for treating Midface Volume Deficiency (MVD) in the US population.

Introduction: Midface volume deficit, a hallmark of facial aging, is characterized by the depletion or shrinkage of supportive tissue in the central facial region. As such, in recent years, an increasing number of patients have sought facial rejuvenation through minimally invasive techniques, with Hyaluronic Acid (HA) dermal fillers being a popular choice.

Materials / method: The primary endpoint was to demonstrate non-inferiority of RHA4 vs active comparator 8 weeks after last treatment (initial or touch-up) for subjects graded 3-4 on the Teoxane Midface Volume Deficit Scale (TMDVS), assessed by the Blinded Live Evaluator (BLE). 200 subjects aged ≥22 years (randomization 3:1) were injected into the subcutaneous to supraperiosteal layers for cheek augmentation. Secondary endpoints included safety assessment, patient satisfaction, GAIS improvement, treatment volumes and optimal cosmetic result (OCR), subject satisfaction scale and midface naturalness.

Results: After 8 weeks, RHA4 led to a clinically meaningful improvement of MVD (≥ 1 grade), as assessed by BLE using the TMDVS. 75% subjects were treated with RHA4 using the Multilayering Approach. To reach OCR, ≤3.5mL was injected on both sides and retreatment volumes were higher in active comparator vs RHA4 (2.3mL vs 1.3mL). RHA4 required fewer treatment sessions vs comparator to achieve similar results. Subject satisfaction with RHA4 remained high throughout the study (over 82%). 88% subjects reported natural looking results in the midface after 2 months. RHA4 was well tolerated for MVD treatment.

Conclusion: RHA4 demonstrated comparable effectiveness to US best in class rated volumizer, achieving similar aesthetic outcomes with fewer treatment sessions. Its safety profile remains favorable regardless of the injection technique used, including the Multilayering approach which was preferred by treating investigators. In addition, RHA 4 delivered natural-looking outcomes. These findings underline RHA4 as an effective solution for midface volume restoration.

Objectives: This study examined the synergistic effect for midface improvement, when pairing poly-L-lactic acid (PLLA-SCA) with skincare through line-field confocal optical coherence tomography (LC-OCT).

Introduction: Aesthetic injectables paired with skincare can improve clinical outcomes and satisfaction. PLLA-SCA regenerates collagen and elastin to correct facial wrinkles and folds. The topical peptide cream (Alastin Skincare® Restorative Skin Complex with TriHex Technology (RSC)) is designed to maintain skin health by recycling the extracellular matrix and modulating adipocytes.

Materials / method: This was a randomized, double-blind, split-face, comparative study where 21 subjects applied RSC on one side of the face and a comparator product on the other side, two weeks before baseline and throughout the study. Subjects were treated with PLLA-SCA in both cheeks at baseline and week 6. LC-OCT imaging was taken at 2 weeks before baseline, and weeks 6 and 18. Skin parameter changes were assessed using Cherry Imaging. Clinical grading of global facial photodamage, tactile roughness at the cheek region, and radiance at the cheek region by investigator and subject questionnaires were gathered.

Results: Continued and directional improvement of dermal fibers was observed following PLLA-SCA and RSC treatment. Greater volume change and subject satisfaction were observed on the PLLA-SCA and RSC side. Clinical grading demonstrated PLLA-SCA treatment paired with either skincare regimen resulted in a statistically significant improvement in skin quality. One adverse event (mild) was related to skincare treatment.

Conclusion: Findings suggest synergistic effects supported by clinical grading, Cherry analysis and questionnaires for improving aging skin and patient experience when pairing PLLA-SCA with RSC.

The RHA line, created with PNT, improves results in dynamic facial areas. With varying HA concentrations, RHA fillers allow customization for specific areas, multilayering, superficial lip injections and contouring. Reversible, versatile, and well-tolerated, HA fillers are growing in popularity, emphasizing the need for practitioner education to guide treatment choices and avoid misinformation.

Materials / method: A search was performed on mainstream social media platforms to identify the most frequent misinformation surrounding the use of HA soft-tissue fillers and a literature review was performed by the authors to achieve a consensus aimed at dispelling each misconception based on an evidence-based approach. The following misconceptions around HA fillers were identified and the authors provided their expert opinions: HAs migrate in the tissues, are permanent, non-reversible and unsafe.

Results: No scientific evidence supports HA migration, which may occur due to poor injection technique, pressure, or incorrect product choice. HA fillers are not permanent, as they are reabsorbed over time through enzymatic degradation. PNT fillers last 6–18 months depending on product modification grade, technique, patient metabolism, and lifestyle. HA reversibility is due to hyaluronidase action. The required concentration depends on HA particle size, crosslinking and G’. BDDE-crosslinked HA fillers have shown safety and efficacy in clinical trials, with most adverse events being mild and transient.

Conclusion: The growth and evolution of soft-tissue fillers for facial rejuvenation has been fuelled by innovation in filler technology. Aesthetic practitioners have a range of products for the same indication, despite different rheological and physicochemical properties. As these options continue to expand, it is key for practitioners to understand the properties of each product and its behaviour towards the dynamic environment of the face to make an informed selection and achieve optimal aesthetic outcomes.

Objectives: Rheology-Driven Embolic Risk: Fillers with higher tan(δ) produce fewer but larger fragments that are less likely to cause proximal occlusion, whereas those with lower tan(δ) produce more, smaller fragments that more readily block vessels.
Dilution Effects in CaHA: Hyperdiluting calcium hydroxylapatite (Radiesse) significantly decreases fragment size and lowers the risk of vascular occlusion compared to undiluted formulations.
Flow Rate Insights: Although flow rate doesn’t greatly affect fragment size, lower flow conditions increase the risk of macrovascular occlusion.

Introduction: Accidental intravascular injection of dermal fillers, including hyaluronic acid (HA) and calcium hydroxylapatite (CaHA), can result in arterial embolization, causing complications such as ischemia, blindness, and stroke. Understanding how these fillers behave in arterial environments is crucial for prevention, management, and advancing knowledge of embolic events. This study established a pulsatile arterial model (PULSAR) to examine the intravascular characteristics of standard, dilute, and hyperdilute CaHA mixtures, alongside multiple HA fillers with varied rheological profiles.

Materials / method: The PULSAR system simulates pulsatile facial artery flow with a 3D-printed polydimethylsiloxane circuit and a modified cardiac pump, preserving physiological pressures and flow rates (~7.4, ~17.6, ~35.8 ml/min). Four HA fillers (Belotero® Intense, Belotero® Revive, RHA4, Redensity) and four CaHA (Radiesse®) mixtures (undiluted, 1:0.5, 1:1, 1:2) were injected (0.2 ml) via a 27G microcannula at 0.01 ml/sec. High-resolution videography captured dispersal, and ImageJ measured fragment size and morphology.

Results: All tested HA fillers formed extensive fragments ranging from 50µm-1mm upon injection, with higher tan(δ) products creating fewer, larger strands that rarely caused proximal occlusion. Lower tan(δ) variants produced more numerous, smaller fragments that more frequently induced vascular blockage. For CaHA, particle size dropped markedly from 0.151mm² in undiluted form to 0.019mm² at a 1:2 dilution, reflecting increased fragmentation. Undiluted CaHA caused complete flow obstruction at low and medium rates, while hyperdilute forms did not occlude at any flow rates. All fillers dispersed distally.

Conclusion: The PULSAR model simulates arterial flow, enabling comparative analysis of HA and CaHA-CMC fillers' embolic behavior in vitro. Gel viscoelasticity and dilution influenced fragmentation and occlusion risk. Flow rates did not alter fragment size but affected macrovascular blockage frequency. High tan(δ) HA and diluted CaHA-CMC likely lower proximal plug formation and reduce distal fragment load. Vessels may occlude via microemboli, suggesting embolic load may outweigh proximal plugs in determining ischemic injury.

Objectives: To evaluate long term aesthetic improvement and subject satisfaction over 12 months following a single dose of relabotulinumtoxinA (relaBoNT-A) in subjects with moderate-to-severe glabellar lines (GLs).

Introduction: RelaBoNT-A is a novel, complexing-protein-free, type-A1 botulinum toxin manufactured using PEARL(TM) Technology as a ready-to-use liquid for aesthetic use. It is approved under the brand name RelfydessTM in some markets in Europe and in Australia. In Phase III trials of GL and lateral canthal line treatment, relaBoNT-A has shown rapid onset (starting from Day 1) and durable efficacy (a median time to return to baseline severity of ~6 months or longer), and the treatment has been well tolerated.

Materials / method: In this multicenter, Phase IIIb, double-blind, placebo-controlled study (NCT05146999), subjects with moderate-to-severe GLs were randomized 3:1 to treatment with 50U relaBoNT-A or placebo. Subjects were followed up for 12 months after treatment. Assessments included subject-reported aesthetic improvement on the 7-point Global Aesthetic Improvement Scale (GAIS) at maximum frown, GL severity by investigator- and subject- live assessment (ILA and SLA) at maximum frown, subject satisfaction with treatment and social function (multiple questionnaires), and collection of adverse events.

Results: RelaBoNT-A (N=99) was superior to placebo (N=33) in terms of GAIS responders at Month 1 (primary endpoint; 92.9% vs 9.1%, p<0.001). GAIS response remained high through Months 9 (43%) and 12 (38%) for relaBoNT-A. GL severity was improved over baseline through Month 9 on the ILA (28%) or Month 12 on the SLA (30%), and median time to return to baseline on ILA/SLA was 9 months. Subjects loved their treatment results at Month 1 (86%) and Month 12 (56%) and >50% of subjects reported increased self-confidence through Month 12. Treatment-related adverse events, all mild, occurred in 8.1% of subj

Conclusion: Efficacy in terms of subject-assessed aesthetic improvement and investigator-assessed GL severity improvement lasted through Months 9 to 12. Subjects reported high satisfaction at Month 1 and remained satisfied through Month 12.

Objectives: The aim of these studies was to develop mesotherapy-inspired technology to improve skin revitalization, through enhancing delivery of ingredients known to restore youthful, healthy skin.

Introduction: Skin is a multi-layered tissue, which protects internal organs from external aggressors, prevents loss of water, regulates temperature, etc. As we age, skin loses its integrity and vitality, manifested as loss of elasticity, firmness and reduced skin barrier function, among other consequences. Mesotherapy or revitalizing injections are known in aesthetic regenerative medicine to improve skin quality, firmness, radiance and youthfulness. An innovative technology has been developed to stimulate skin revitalization by enhancing key mesotherapy ingredients and their delivery.

Materials / method: Key core ingredients were identified and placed in a simple base formula for efficacy assessment. Skin explants, EpiDermFT and EpiDerm were utilized to evaluate biomarkers and delivery. Gene expression was analyzed by QPCR, and protein expression was analyzed with ELISAs, immunofluorescence, or proteomics. Mass spectrometry imaging and Franz cell diffusion were utilized for delivery assessments.

Results: Mesotherapy-inspired ingredients were selected for skin benefits. Chronosphere encapsulation and penetration enhancement technology ensured maximum delivery of ingredients to tissues. Gene expression analysis or protein analysis revealed that the NCEF 2.0 technology stimulated skin barrier and extracellular matrix biomarkers, while improving antioxidant regulation and vitality. Additionally, proteomics analysis of the NCEF 2.0 technology demonstrated upregulation of 118 proteins, while downregulating 31 proteins.

Conclusion: Mesotherapy-inspired technology, NCEF 2.0, reverses the signs of skin aging through revitalization. With enhanced delivery of key ingredients, NCEF 2.0 has been shown to improve antioxidant protection, energy production, renewal of extracellular matrix, skin integrity and barrier improvement, and activation of cell metabolism.

Objectives: To provide a comprehensive assessment of the immunogenic profile of RelabotulinumtoxinA (relaBoNT-A) after single-dose and repeated injections for glabellar line (GL) and lateral canthal line (LCL) treatment.

Introduction: All therapeutic proteins, including botulinum toxin A (BoNT-A), can potentially induce antidrug antibodies (ADA). ADAs that bind to a drug product to neutralize its desired effect are termed neutralizing antibodies (NAb) and are important to assess during development of a new product. RelaBoNT-A is a new, highly active, complexing-protein-free, innovative, ready-to-use liquid BoNT-A, developed from a proprietary Clostridium botulinum strain. It is approved in some markets, under the brand name Relfydess™, for aesthetic improvement of moderate-to-severe GLs and LCLs.

Materials / method: 1847 subjects (~50% with prior BoNT-A experience) were treated with ≥50U relaBoNT-A in the GL and/or LCL area in 1 of 5 single-dose clinical trials Ph I-III, or in a Ph III repeat-dose long-term safety study. Subjects in the long-term study received up to 4 repeat treatments per GL/LCL area. Blood (serum) samples from baseline and post-treatment were analyzed for ADA against relaBoNT-A using ELISA, (3 assays in a multi-tiered approach) and ADA+ samples were then analyzed in a mouse protection assay (MPA) for NAb. Treatment-emergent adverse events (TEAEs) among ADA+ subjects were evaluated.

Results: Out of 1699 ADA evaluable subjects (of which 754 received multiple relaBoNT-A doses), 1.3% of subjects were confirmed in the ELISA assays as ADA+ at any timepoint and 1.1% were treatment-emergent ADA+. All ADA+ samples were negative for NAb in the MPA test, indicating absence of neutralizing ADA. No differences in TEAEs were observed in ADA+ and ADA- subjects. TEAEs related to the study product reported in ADA+ subjects were mild-to-moderate in severity and non-serious. No serious or severe immune-related TEAEs were observed.

Conclusion: Based on analysis of 1699 subjects treated with relaBoNT-A, of which 754 had multiple relaBoNT-A treatments (up to 4 repeat treatments in each indication) and ~50% had prior BoNT-A experience, relaBoNT-A treatment does not lead to the formation of neutralizing antibodies. This demonstrated low immunogenic risk following treatment with the complexing-protein-free relaBoNT-A formulation.

Objectives: To evaluate the perceived naturalness of facial expression and dynamic facial movement following treatment of nasolabial folds and/or marionette lines in the lower face with two Restylane® OBTTM hyaluronic acid (HA) fillers (Restylane® Refyne™, Restylane® Defyne™) utilizing data from three Phase IV clinical studies.

Introduction: Natural facial expressions are an important aspect of nonverbal communication, and patients seeking facial rejuvenation want discreet and natural‐looking treatment results. The OBT™ technology produces flexible HA fillers that return to their original shape after deformation, making them ideal for treating dynamic facial areas where strain varies with facial movements. This analysis evaluated the naturalness of facial expression following correction of nasolabial folds or bilateral marionette lines with Restylane Refyne and/or Restylane Defyne.

Materials / method: from three Phase IV clinical studies involving Restylane Refyne and Restylane Defyne were presented by study and compared descriptively. In all three studies, subjects received either Restylane Refyne or Restylane Defyne for the treatment of nasolabial folds and/or marionette lines and were assessed at 4–8 weeks or 6 months following treatment by the investigator and an independent reviewer regards the naturalness of facial expression and dynamic facial movement using photographs, two-dimensional video recording, assessment scales and questionnaires at baseline and after treatment.

Results: In one study, >80% of subjects maintained natural facial expressions 8 weeks after Restylane Refyne or Restylane Defyne treatment in the marionette lines and nasolabial folds, as assessed by an independent reviewer. In another study, investigator-assessed naturalness of facial expression was enhanced or maintained in ≥95% and ≥98% of subjects at 1 and 6 months, respectively, after Restylane Refyne or Restylane Defyne. In a third study, investigator-assessed naturalness of facial expression was enhanced in 80% and maintained in 20% of subjects 6 weeks after Restylane Refyne or Restylane Defyne.

Conclusion: The naturalness of facial expression, both when relaxed and dynamic, was preserved while achieving desired aesthetic improvements in attractiveness and youthfulness using Restylane Refyne and Restylane Defyne in three phase IV clinical studies. This naturalness of facial expression, combined with high patient and investigator satisfaction, represented a long-lasting aesthetic improvement. Healthcare practitioners should aim to maintain the naturalness of facial expression when treating areas of facial animation such as the nasolabial folds and marionette lines.

Objectives: This case series evaluates the therapeutic outcomes of using the Tri-Wave MD® (Dermalux) device in treating four patients with rosacea, exploring the efficacy of blue, red and near infrared (NIR) LED light combination phototherapy.

Introduction: LED phototherapy has shown promise in treating various skin conditions, though more evidence is needed for its efficacy in rosacea. This case series presents the outcomes for four rosacea patients treated with the Tri-Wave MD® (Dermalux) device using blue, red and NIR LED light therapy.

Materials / method: Four female patients (36–59 years) with rosacea and Fitzpatrick skin types II–III, some with other inflammatory co-existing conditions such as acne or psoriasis, were treated with the Tri-Wave MD® device (Dermalux). Treatment was weekly for 20 mins/week over 6 weeks using a combination of blue, red and NIR light. Each patient received a personalised treatment plan with different wavelengths and energy.

Results: All patients experienced improvements in overall skin quality, decreased inflammation and diminished pigmentation. A patient with significant hyperpigmentation due to photo damage reported both reduced pigmentation and skin tightening. All four patients were satisfied with the outcomes, reporting enhanced self-confidence and overall mood.

Conclusion: To the best of our knowledge, this is the first documented case series of patients with rosacea treated successfully with combination LED phototherapy. It suggests that combination LED phototherapy is effective in treating inflammatory skin conditions, with the added secondary benefit of improving patients’ self-confidence and mood.

Objectives: To evaluate the in-vivo local tissue response and degradation of two poly L-lactic acid biostimulators (Sculptra® [PLLA-SCA] and Gana V® [PLLA-GA]) in relation to differences observed in the clinical outcomes of the two products.

Introduction: Biostimulatory injectables utilizing PLLA have similar formulations but marked differences in everyday clinical practice performance. PLLA-SCA has a more amorphous and less crystalline structure than some PLLA injectables and comprises a homogenous formulation, which is reflected in its slower degradation rate and longer preservation of mechanical strength and translates clinically as a longer duration of effect. This in-vivo study evaluated the local tissue response and degradation following subcutaneous implantation of PLLA-SCA and PLLA-GA.

Materials / method: Thirty-two rabbits were implanted subcutaneously with 0.2 mL of either PLLA-SCA, PLLA-GA (both reconstituted as per manufacturer’s instructions) or negative control on the dorsal surface. At 4 days (baseline) and at 2, 12 and 26 weeks, the tissue at the implant sites and regional lymph nodes were collected and processed for histopathology to evaluate local tissue response and product degradation. Evidence of local tissue response was determined using a microscopic reactivity score compared with the negative control, and persistence (degradation) was evaluated using a semi-quantitative scale.

Results: Differences in average reactivity scores between test and control groups indicated a slight reaction for PLLA-SCA at all time points evaluated (3.1, 6.2 and 6.0 at 2, 12 and 26 weeks, respectively), while for PLLA-GA a moderate reaction was observed at 12 and 26 weeks (2.3, 9.7 and 9.2 at 2, 12 and 26 weeks, respectively). PLLA-SCA and PLLA-GA had average persistence scores of 2.6 and 2.0, respectively, indicating faster degradation with PLLA-GA than PLLA-SCA. Histologically, there was no evidence of PLLA-SCA, PLLA-GA or any degradation products in the draining lymph nodes over 26 weeks.

Conclusion: These results suggest that the in vivo local inflammatory tissue response observed with PLLA-GA is greater and the degradation rate is faster than PLLA-SCA. The increased inflammation seen with PLLA-GA may contribute towards the worse adverse event profile reported clinically, while the faster degradation rate could lead to increased lactic acid levels, the main degradation product, creating a more toxic local environment, as well as reducing the duration of clinical effect. Differences in formulation between PLLA-SCA and PLLA-GA can have considerable impact on clinical outcomes.
